Thank you for your application.  We will begin application processing as soon as we receive the required certification. 
 
Upload a scan of your FLETC Graduation Certificate, or RTP Certificate following form submission process.

Alternatively, email a copy of your Certificate to membership@ffdoa.org, but the preferred method is to upload it to our site with the above form link.
  1. We will verify your eligibility and you will receive an email notifying you of approval.  You cannot log into the web site until your credentials have been verified!
  2. We are all volunteers, so please allow several days for us to process your application and verify your eligibility.
* You may fax your FLETC Graduation Certificate to 214-594-7345.  If faxing, a cover sheet must be attached which includes the email address that you will be providing in this application.  If the fax and/or cover sheet are illegible, we may not be able to identify and reply to you.  Therefore, if you do not hear from us within two weeks of faxing, please contact us at support@ffdoa.org.
